Partnering with providers, health plans, and life sciences companies, including CMSâ GUIDE dementia care model, Synapticure is dedicated to transforming the lives of millions of individuals and their families living with neurodegenerative diseases.
The Behavioral Health Care Manager is a support role addressing the mental health needs of individuals and their families affected by neurodegenerative diseases. Working under clinical oversight, you will provide care coordination and behavioral health support to patients, caregivers, and their loved ones while implementing Synapticureâs Collaborative Mental Health Care (CoCM) program. This role involves working closely with senior care managers, medical providers, patients, and partners to drive meaningful change and ensure exceptional mental health care for this community.
$65,000 - $70,000 a year
Final compensation will be determined based on location, experience, and qualifications.
