Requirements:

PA or NP from accredited program

Board Certification (NP: ANCC, PA: PA-C)

Bilingual (Spanish - read, write, and speak).  Must be able to successfully complete a fluency assessment (verbal and written).

Current full practice licensure in at least 3 of the following states: FL, TX, OH, NJ, GA (active at the time of submitting application)

Willingness to be licensed in additional states with our assistance, working with our licensing vendor

3+ years experience in primary care, urgent care, ER or outpatient family medicine practice

2+ years of telehealth experience

Pay Transparency:

Responsibilities:

  • Provide high quality care for patients in a virtual setting
  • Collaborate with Physicians and/or Advanced Practice Leads on the Care Team as needed when working with complex patients
  • Follow up on labs, imaging, and closed loop communication with patients and other providers as needed
  • Use tooling to allow virtual “exams” including remote home monitoring equipment, remote physical exam, remote point of care diagnostics
  • Participate in collaborative care model with various care team members including but not limited to registered nurses, medical assistants, PCPs, behavioral health clinicians, and clinical supervisor where applicable
  • Participate in required meetings to support patient care including huddles, case conferencing and regular service line All Hands
  • Provide feedback to support improvement in operational workflows and product features

Requirements:

  • MD/DO from accredited program
  • Board Certification (ER/FM/IM)
  • Spanish fluency (read, write, and speak)
  • 3+ years of outpatient urgent care or emergency department experience
  • 2+ years of experience delivering virtual care/telemedicine
  • TN and FL active IMLC – preferred with active LOQ
  • Willingness to be licensed in additional states with our assistance, working with our licensing vendor
  • Compact license from IMLCC or eligible for a compact license (hold a full, unrestricted medical license in a state that is a member of the Compact)

About the Role

The Behavioral Health Care Manager is a support role addressing the mental health needs of individuals and their families affected by neurodegenerative diseases. Working under clinical oversight, you will provide care coordination and behavioral health support to patients, caregivers, and their loved ones while implementing Synapticure’s Collaborative Mental Health Care (CoCM) program. This role involves working closely with senior care managers, medical providers, patients, and partners to drive meaningful change and ensure exceptional mental health care for this community.

The Day to Day

  • Fulfill foundational responsibilities of the Behavioral Health Care Manager (BHCM) role within the Collaborative Care Model (CoCM) program, adhering to the guidelines established by the University of Washington AIMS Center.
  • Support the mental and physical health care of assigned patients, rendering coordination with their medical providers and other mental health professionals.
  • Screen and assess patients for mental health disorders, facilitating engagement and follow-up care.
  • Provide education to patients and caregivers about mental health disorders, including available treatment options.
  • Monitor clinical symptoms and treatment outcomes, documenting progress and adapting care plans under supervision as needed.

  • Passion for working with vulnerable populations and providing patient-centered care.

Preferred Qualifications

About the Role

This role is a 1099 contract role, targeting 10-20 hours per week.

We are seeking a compassionate, collaborative Speech-Language Pathologist (SLP) to provide assessment, intervention, and longitudinal care for adults with progressive neurodegenerative conditions across a fully remote, telehealth-based model.

This role focuses primarily on direct patient care, including evaluation, evidence-based treatment, caregiver education, and interdisciplinary collaboration. Working closely with senior clinical team members, the SLP will manage a varied caseload of patients with Parkinson’s disease, ALS, Huntington’s disease, FTD/PPA variants, and related neurologic conditions, adapting clinical care to disease progression and patient goals.

The Day-to-Day

Assessment & Diagnosis

  • Conduct virtual speech, language, cognitive-communication, motor speech, voice, and dysphagia risk evaluations using standardized assessments and functional task analysis adapted for telehealth.
  • Interpret findings to establish diagnostic impressions, identify functional goals, and formulate measurable, patient-centered treatment plans.
  • Recognize when additional assessment, referral, or consultation with the Senior SLP is warranted, including when a patient’s needs extend beyond the scope of telehealth services.

Treatment & Intervention

  • Deliver stage-appropriate, evidence-based interventions for progressive neurologic conditions, including:
    • Cognitive-communication therapy targeting functional daily tasks, memory, and executive function.
    • Motor speech therapy and compensatory communication techniques.
    • Voice banking, message banking, and early communication-preservation planning.
    • Augmentative and Alternative Communication (AAC) evaluation, trial support, and caregiver training.
    • Telehealth-appropriate dysphagia screening, education, compensatory strategy training, and referral for instrumental assessment when indicated.
  • Adapt intervention plans as clinical needs, functional priorities, and disease stages shift over time.
  • Educate caregivers and family members on communication strategies, cueing techniques, and environmental modifications to ensure skill carryover.

Collaboration & Team Integration

  • Partner with neurologists, care coordinators, behavioral health providers, and senior SLPs to align on care priorities and manage complex cases.
  • Participate in interdisciplinary case conferences, clinical consultation, and case review as appropriate.

Documentation & Compliance

  • Maintain timely, accurate, and confidential clinical documentation in the EMR in compliance with ASHA standards, HIPAA, state licensure rules, and payer requirements.
  • Complete evaluations, session notes, progress reports, and AAC funding packets within required timelines.

Minimum Qualifications

  • Master’s degree (M.A. or M.S.) in Speech-Language Pathology from an accredited program.
  • Certificate of Clinical Competence (CCC-SLP) from ASHA.
  • Active state license (or eligibility for licensure) in at least one US state, with willingness to obtain additional state licenses as needed.
  •  3-5+ years of clinical experience as a licensed SLP, with at least 1-2 years of direct experience working with adult neurodegenerative populations.
  • Proficiency with electronic medical records (EMR) systems, basic AAC technology, and remote/telepractice software.
  • Strong written, verbal, and interpersonal communication skills.

Preferred Qualifications

  • Active certification in LSVT LOUD or SPEAK OUT! (or willingness to complete training within 90 days of hire).
  • Familiarity with multi-state telepractice delivery and virtual service workflows.
  • Exposure to AAC evaluation process, including device trials and funding documentation.

Summary of Role: In this Clinical Research Associate role, you will be responsible for monitoring the progress of clinical studies, either at investigative sites or remotely, and ensure clinical trials are conducted, recorded, and reported in accordance with the protocol, standard operating procedures (SOPs), ICH-GCP, and all applicable regulatory requirements.

We are recruiting for two roles: one based on the West Coast and one based on the East Coast. Travel expected: ~1+ site visit per week, for sites that do not permit virtual monitoring.

Responsibilities:

  • May serve as the primary monitor and main point of contact for assigned clinical sites
  • Conduct interim monitoring visits (IMVs) and ISF review
  • Review electronic medical records (EMR)/patient data
  • Source documentation verification (SDV) against case report form data, including informing the site staff of any entry errors, ensuring good documentation practices are being adhered to and communicating protocol deviations appropriately per SOPs, GCP and applicable regulatory requirements
  • Verification and review of adverse events, serious adverse events, concomitant medications and corresponding illnesses to confirm accurate data reporting in accordance with the protocol
  • Provide support to site staff including research coordinators and physicians
  • Assist in the development of trial materials as needed (e.g., case report forms, monitoring guidelines, site tools and training guides)
  • Contribute to the development of Clinical Operations SOPs, best practices, work instructions, tools and templates
  • Author and submit high-quality monitoring visit reports within required timelines
  • Ensure completeness and accuracy of monitoring documentation in the eTMF

Qualifications:

  • BA/BS or equivalent with a minimum of 2+ years of CRA experience (for CRA role) or 5+ years with lead monitor responsibilities (Senior CRA)
  • Therapeutic experience in oncology/hematology and cell and gene therapy preferred
  • Prior CRA experience working directly for sponsor, ideally a start-up/or small company, with primary site ownership and independent monitoring responsibility
  • Strong working knowledge of FDA and ICH/GCP regulations and guidelines and thorough knowledge of clinical monitoring procedures
  • Experience with Trial Master File management according to the DIA reference model
  • Experience operating within various site EMR/EHR systems
  • Experience using eTMF, EDC (IBM Zelta experience ideal), CTMS, and other vendors systems/portals, as well as working on trials without a CTMS in place

$90,000 - $140,000 a year

The anticipated annual salary range (listed above) for this job is based on prior experience in the role/industry, education, location, internal equity, and other job-related factors as permitted by law. For remote-based positions, this range may vary based on your local market. Full-time employment positions will also be eligible to receive pre-IPO equity and annual bonus, in addition to competitive medical, dental, and vision benefits, PTO, 401(k) plan, life and accidental death and disability coverage, and parental leave benefits.
